SiteOrigin Contact form not fully responsive?

Hi team

There’s 1 thing that doesn’t seem to be fully responsive on my site, and that is the SiteOrigin Contact form. It came with the SiteWidget Bundle recommended to use with the Virtue theme.
When you look at this page, the contact form near the bottom doesn’t look right. It seems to only look weird on a smart phone in portrait format. Landscape is fine.

Is there something that I’m missing or doing wrong? It seems strange that this doesn’t work properly. I’ve written the developers before but it usually takes over 1 month for them to respond unless you buy their premium package, which I don’t actually need… I searched online but couldn’t find any info on this.

I looked on http://www.responsinator.com to check this. I also looked on my own iphone, and it looks as weird.
