How to replace title text with that of a responsive image?

Hi,
I’m sure it has been asked before, but here it goes my stupid question:
1. Is it possible to replace the title text (H1, right) on a page manually with that of a responsive image? On that place? Left aligned.
2. If so, how to maintain full SEO benefit of having a written H1 title at the top place (instead that of an image, say with, title=””)? I guess the benefit is gone, then!?
