Not receiving email notification from form

Hi there,
I’m using Kadence Theme/Blocks/Pro and have several forms, but when I send a test to myself I’m not receiving any email notifications. When I first set up the first form a couple of months ago, that seemed to work so I’m not sure what’s changed since then. Can you provide any insight on what I may be doing wrong? The site is JD Chapman test website.
